Phimosis - symptoms
How to recognize a problem with phimosis? In children up to the age of two, it is normal for the foreskin to stick to the head of the penis. The foreskin is stuck to the glans with a physiological sticky discharge (the so-called mastka), the amount of which gradually decreases in the following months. In a three-year-old, the foreskin should be completely removable. The parent can perform a simple examination of the child, gently checking the mobility and scope of the foreskin retraction. It is recommended to perform the test while bathing and when the child is relaxed.
In adult men, the problem of phimosis is diagnosed when the foreskin cannot be completely removed and the glans exposed - this applies to the situation when the penis is at rest or when the penis is erect. Sexual intercourse may be accompanied by the pain of tightening the penis by phimosis. In the event of inflammation, the area of the glans may be additionally red and painful, even when at rest.
The causes of phimosis
Phimosis can be a congenital lesion of the penis. However, the most common cause of phimosis in young boys is inadequate care of the penis in infancy. The residual urine and other impurities are conducive to bacterial infections and inflammation causing secondary shrinkage of the foreskin. The opening of the foreskin becomes smaller and smaller, so it cannot be effectively drained away. Similarly, in adults, neglecting hygiene increases the risk of chronic inflammation and scarring that reduces the flexibility of the foreskin.
Complications of untreated phimosis
Persistence of phimosis in a child over 3 years of age may inhibit the development of the penis. The narrowing of the foreskin can also make it difficult to urinate freely. In adult men, a narrowed foreskin may limit sperm ejaculation and cause erectile dysfunction. Long-term pressure and superinfection lead to chronic penis inflammation. The long-term effect of neglecting the problem is an increased risk of malignant neoplasm, i.e. penile cancer.
Phimosis - treatment
Treatment of the early stages of phimosis in children is to regularly stretch the foreskin. This should be done with care so as not to damage the foreskin or jam it. If necessary, local anti-inflammatory agents are used. Taking care of the proper care of the penis prevents the problem from recurring.
Phimosis surgery - when is it necessary?
When stretching the foreskin remains ineffective, and there are symptoms associated with obstructed outflow of urine, erection problems or recurrent inflammation, it is advisable to perform foreskin surgery.
Phimosis treatment - what is it?
The foreskin surgery may consist in enlarging its mouth (plasty of the foreskin) or in its partial or complete removal (circumcision). The phimosis removal procedure is completely painless - it is performed under general anesthesia for children or under local anesthesia for adults. If necessary, self-absorbing sutures are applied, which dissolve spontaneously after about 2 weeks (they do not need to be removed afterwards). The treatment site may be provided with an antibiotic.
Foreskin surgery is a minimally invasive and relatively simple procedure. The surgeon's experience in this area allows for a good cosmetic effect. Convalescence is quick and easy. After the phimosis removal procedure, you should take care of penile hygiene and avoid staying in high ambient temperatures. After the wound is healed, a small scar should be massaged to maintain the elasticity of the tissues. Within one month of the procedure, it is recommended to refrain from sexual intercourse.
